When you call to book an appointment, you will be sent a COVID-19 screening questionnaire to complete online the day before your appointment. The form will be reviewed by a member of the clinical team, and it may be necessary to reschedule your appointment should you, or a member of your household, develop any symptoms of COVID-19.
